Dr. Williams is currently offering a Functional Medicine Case Review option for patients who are not yet ready to dive straight into Functional Medicine testing and treatments, but want to investigate how a Functional Medicine approach would be different from what conventional medicine has been able to provide to them thus far.
TYPES OF PATIENTS WHO MAY BENEFIT FROM A FUNCTIONAL MEDICINE CASE REVIEW:
1. Patients who are frustrated by being prescribed medications to cover-up or lesson symptoms, but have not been given any real answers, or even a willingness to investigate, what may have caused their condition(s) to arise in the first place.
2. Patients who feel unheard by their healthcare provider(s), or who have been made to feel like "it's all just in their head", simply because a few very basic tests are still coming back "within normal limits". However, it is important to understand that you can be at the very top or very bottom of those "normal limits" and still be experiencing all the symptoms associated with that condition whether you have technically reached that magical threshold number or not.
3. Patients who want a Doctor who will take a comprehensive view of their case. A Doctor who will compile and examine their medical records from all providers they have seen over the past 10 years, one who will review their lab trends over the past 10 years, and one who will take an in-depth look at their history, environmental exposures, diet, and any other contributing factors that may be affecting their current health status.
HOW A FUNCTIONAL MEDICINE CASE REVIEW WORKS:
1. Call the office and ask for a Functional Medicine Case Review. A time will be set to meet with Susan, Dr. Williams Functional Medicine Assistant, to go over how to fill out the extensive history packet questionnaire and 7 day food diary. There is no charge for this visit which will last between 45-60 minutes.
2. Fill out the packet making certain to answer every question and to provide to our office the names, addresses, phone and fax numbers for all providers you have seen over the past 10 years and complete the 7 day food diary.
3. Call the office again to schedule a time to drop off the completed history packet and 7 day food diary. At this time, the patient will sign medical record request forms and pay the non-refundable $250.00 compilation fee(we accept cash only for this payment, a receipt will be given) and our office will begin requesting the patients medical records for Dr. Williams to review.
4. Once our office has received all requested medical records, someone will phone the patient to schedule their one hour consultation with Dr. Williams. At this visit, the patient will pay the additional $150.00 Consultation fee (we accept cash only for this payment, a receipt will be given). Please note: if for any reason we do not receive any of the requested medical records within 2 weeks of our office requesting them, the office will phone the patient to give them an opportunity to collect those records themselves and deliver them to our office. We will not make a second request, it is ultimately the patient's responsibility to make certain all medical records are delivered to our office for Dr. Williams to review. Dr. Williams will not review any of the medical records until he has all of the medical records because they need to be read in conjunction with each other, providing a roadmap to the patient's current health status.
WHAT IS INCLUDED IN THE FUNCTIONAL MEDICINE CASE REVIEW:
1. Dr. Williams will review the patient's completed history packet and 7 day food diary.
2. Dr. Williams will review all medical records provided by all physicians and hospitals for the past 10 years.
3. Dr. Williams will review all lab results provided for the past 10 years.
4. Dr. Williams will give written recommendations regarding the types of additional laboratory testing that may be helpful in determining the root cause of your condition(s) based upon your case review and additional recommendations as to possible diet and lifestyle changes that may be beneficial. The patient will have a one hour (limited to 60 minutes maximum) consultation with Dr. Williams where they can discuss their current health conditions and Dr. Williams will explain how Functional Medicine would approach these conditions.
PLEASE NOTE: THIS IS A CASE REVIEW AND CONSULTATION ONLY, no actual lab orders will be given to the patient, and no recommendations will be made as to any supplements/nutraceuticals to be taken by the patient as these can only be determined once Functional Medicine testing has been completed and analyzed.
The total Functional Medicine Case Review fee is $400.00 (broken into 2 payments, a $250.00 non-refundable compilation fee and a $150.00 consultation fee).
WHAT IF AFTER MY CONSULTATION I DECIDE I WANT TO BECOME A FUNCTIONAL MEDICINE PATIENT, HOW DO I PROCEED?
If within 30 days of the Functional Medicine Case Review consultation appointment, the patient decides they want to become a Functional Medicine patient with Dr. Williams, the fee for the Functional Medicine Examination and required testing orders will only be $100.00 payable on the day of their Functional Medicine Examination Appointment (this does not include laboratory testing fees as these are always paid by the patient directly to whichever lab(s) are running the tests). There will be no additional charge to return for the results of their Functional Medicine testing. If between 31 and 90 days from the date of the Functional Medicine Case Review consultation appointment, the patient decides they want to become a Functional Medicine patient with Dr. Williams, the fee for the Functional Medicine Examination and required testing orders will be $250.00 payable on the day of their Functional Medicine Examination Appointment (this does not include laboratory testing fees as these are always paid by the patient directly to whichever lab(s) are running the tests). There will be no additional charge to return for the results of their Functional Medicine testing. After 90 days from the date of the Functional Medicine Case Review consultation appointment, the patient will have to begin the new patient process all over again and the fees for the first visit with Dr. Williams will return to $500.00.
